In an emergency, the Network is required by the Centers for Medicare & Medicaid Services to:

• Work with Federal, State and local government agencies as needed to assist with
patient safety and ensure dialysis facilities are prioritized to be open
• Assist with, track, and report patient access to treatment
• Report and publish the status of facility operations during emergency events

Annual Critical Assets Survey (CAS)

  1. Complete the CAS worksheet to ensure that you have the most accurate data for your facility.
  2. Visit https://isweb.ipro.org/esrd_cas/login.aspx to log in to the online CAS reporting system.  (TIP: Have your facility's CMS Certification Number (CCN) on hand.  All Network reporting requires the facility's six digit CCN (i.e. 33-XXXX)).
  3. There is one login per facility: the login id is the facility CCN Number, the default password is esrd and you will be required to enter your email address.  Once the password of esrd is entered, you will be prompted to create a new password where you will need to enter the facility zip code, the new password and then confirm the new password.
