In an emergency, the Network is required by the Centers for Medicare & Medicaid Services to:

• Work with Federal, State and local government agencies as needed to assist with
patient safety and ensure dialysis facilities are prioritized to be open
• Assist with, track, and report patient access to treatment
• Report and publish the status of facility operations during emergency events

Annual Critical Assets Survey (CAS)

  1. Download and Complete the CAS worksheet to ensure that you have the most accurate data for your facility.
  2. Visit to submit your CAS information.  (TIP: select your county, and then select your facility.  If you facility is NOT listed in the dropdown, there is a field to enter your CCN, Facility Name and county)).
  3.  If you have any difficulty reporting into the CAS, please e-mail
